Important information for people submitting on the submission deadline.
Submission Guidelines
Please review the guidelines below before proceeding with your online submission.
DO NOT PROCEED UNTIL YOU HAVE THE SUBMISSION REQUIREMENTS SPECIFIED ON THE PREVIOUS PAGE.
We recommend that you read through the PDF submission example (found on the previous page) and then type your answers to the detailed submission questions in Word (or some other program). Then you can copy and paste your answers into the question blocks on the following web pages. This program will timeout if you take longer than 30 minutes to complete a page.
NOTE, you cannot save your submission until the end. If you stop part way, you will have to start over. If you have already typed up your answers to the submission questions, it should only take 10 minutes to complete the online submission.
Once you have completed the online submission process the lead presenter will receive an email confirming the submission. If the lead presenter DOES NOT receive an email within 24 hours, you MUST contact Bruce Phillips, 1-800-274-2237, ext 5405 or bphillip@stfm.org, to confirm that your submission was successfully recorded in our database.
