To order a copy of a STFM mailing list for a one-time use, complete the below form and return it to the STFM office with a sample of the mailing piece.  The use of a STFM list is contingent upon the approval of the mailing piece.  

Please allow 1 to 2 weeks for processing.  

Note: E-mail addresses are not available with any list purchase.
