We welcome any volunteers who want to join our advocacy network! If you would like to become an Advocacy Representative and get involved in academic family medicine advocacy, please send the following information to CAFMadvocacy@stfm.org:
- Full name
- Suffix (MD, MPH, etc.)
- Name of the institution with which you are affiliated
- Name and email address of your Program Director/Department Chair
- Email address
- Phone number
- Mailing Address
- Zip code + 4 (a full zip code allows us to determine which member of Congress represents you)
To look up your four digit zip code extension, go to http://zip4.usps.com/zip4/welcome.jsp. Unfortunately, the zip code is not enough to look up your congress person as congressional boundaries do not follow zip code boundaries. But the zip code extension will give us detailed enough information in order to determine your representative.
We look forward to helping you become a voice for family medicine in Washington!
